Compliance With Mine Plan NA - Support Facilities Not On-site <br /> NA -Other NA - Special Categories Of Mining <br /> N -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a complete inspection of the Williams Fork Mines; DRMS Permit No. C-1981-044 operated by Moffat <br /> County Mining, LLC. I, Jared Ebert of the Colorado Division of Reclamation,Mining and Safety(Division) <br /> conducted the inspection. Ms. Christine Belka and Spencer Shumate with the Office of Surface Mining (OSM) <br /> accompanied me during the inspection. Mr. Brian Watterson of Moffat County Mining escorted us during the <br /> inspection. On Tuesday November 19th,the weather was cold and rainy. On Wednesday November 20th it was <br /> partly cloudy and warm. <br /> AVAILABILITY OF RECORDS—Rule 5.02.4(1): <br /> A records check was conducted at the Foidel Creek Mine office on Tuesday November 19th, 2013 details of the <br /> Division's findings are noted at the end of this report. <br /> There were several records that could not be located during the inspection. First,the Division could not locate any <br /> information at the mine office in regards to the SL1 bond release application. Upon returning to the Division <br /> office,the approval letter for this bond release was found. The Division has enclosed a copy of this letter; please <br /> include a copy of this letter in the records binder for future records checks. <br /> Second,the Division could not locate the Underground(UG) Mining Landowner Notification letters within the <br /> files at the mine office. The Division was able to locate two notification letters within our files. These letters are <br /> enclosed; please include them in the records binder. <br /> BACKFILL and GRADING—Rule 4.14 <br /> Contemporaneous Reclamation 4.14.1;Approximate Original Contour 4.14.2;Highwall Elimination 4.14.1(2)(f); <br /> Steep Slopes 4.14.2, 4.27;Handling of Acid and Toxic Materials 4.143; Stabilization of Rills and Gullies 4.14.6: <br /> The reclaimed No. 5 portal area was observed.
