Please provide the Division with signed copies <br />of the agreements when available or provide evidence the appropriate notice was provided <br />to the well owners. <br />The Operators response stated the agreements that have been obtained were attached. <br />The other agreements are being negotiated and certified mail was sent to Kohloff and <br />Taylor. <br />a) Please provide the Division with signed agreements or proof of certified mailing to <br />Kohloff and Taylor. <br />b) Please provide the Division with signed agreements or proof of certified mailing for <br />the remaining well owners within 600 feet of the Loloff Mine as listed in Exhibit G: <br />a. Hoshiko Farms— Permit #12332 <br />b. Midexco Construction— Permit #134824 <br />c. Varra Companies— Permit #75965 <br />d. Aggregate Industries — Permit #63551 <br />c) Exhibit G lists two wells owned by Robert D. Francis, Permits #27480 and #135883; <br />however the agreement submitted between Loloff Construction and Mr. Francis lists <br />three wells, Permits #30562, #135883 and #2655A. Please explain this discrepancy. <br />d) The agreement between Monroe and Loloff Construction is difficult to read. Please <br />submit a legible copy. <br />
