<br />9.A.(4) The engineer shall observe the construction of the <br />dam. It. is the engineer's responsibility to make frequent periodic <br />visits to observe the progress andquallty of the construction to <br />determine whether the construction is proceeding In accordance with <br />the approved plans and specifications. The engineer shall endeavor to <br />prevent defects and deficiencies in the construction of the dam and <br />appurtenant structures, and shall disapprove or reject work failing to <br />conform to the approved plans and specifications. <br /> <br />9.A.(S) The engineer shall maintain a record of con- <br />struction, which as a minimum, shall include: ' <br /> <br />9.A.(S)(a) Daily activity and progress reports; <br /> <br />9.A.(S)(b) All test results pertaining to construction; <br /> <br />9.A.(S)(c) Photographs sufficient to provide a record <br />of foundation conditions and various stages of the construction <br />through completion; <br /> <br />9.A.(S)(d) All geologic information obtained; and <br /> <br />9.A.(S)(e) Construction problems and remedies~ <br /> <br />9.A.(6) A construction progress report summarizing the <br />contents of 9.A.(S)(a), 9.A.(S)(b), and 9.A.(S)(e) above, must be <br />submitted to the State Engineer every 30 days or more frequently if <br />directed by the State Engineer. A summary report of all the Items in <br />Rule 9.A.(S) shall be submitted at the end of cosntructlon. <br /> <br />9.A.(7) The engineer shall give the State Engineer at least <br />five days advance notice of initial materials placement on the dam's <br />foundation, In the cutoff trench, and on the outlet foundation (or any <br />appurtenance requested by the State Engineer in advance), to allow <br />observation by the State Engineer. <br /> <br />9.A.(B) When unforeseen site conditions or material <br />availability require that the construction work differ significantly <br />from the approved plans and specifications. a change order. including <br />details, must be prOVided by the engineer to the State Engineer. No <br />change shall be executed until approved by the State. Engineer. Major <br />changes must be. submitted in writing with supporting documentation, <br />and approved in writing by the State Englener. Minor changes may be <br />transmitted verbally by the engineer and approved by the State <br />Engineer verbally. <br /> <br />. 32 - <br />
