SECTION 4.0 <br />RFP PAA -313S <br />SUBMITTAL <br />4.1 INTRODUCTION: <br />This section describes the information which must be furnished by the proposer and the prescribed <br />format in which it must be presented <br />4.2 RESPONSE SCOPE AND FORMAT: <br />n order o e responsive o is ,the proposer must fully address each item of this section (4.3 <br />through 4.7). The proposer must start with the first item and consecutively continue through completion <br />of all items. The procedure for response is as follows: <br />4.2.1 Identify the item by the paragraph representation which references it, e.g. 4.3, 4,5.2, 4.5.3, etc. <br />4.2.2 Fully,address the subject item. The description may be in narrative or outline format. Be as <br />concise as you can wile discussing the items fully. <br />4.2.3 Reference and attach any supporting documents that help describe or contribute to an item. <br />Reference to technical literature can be given in support, but not in place of, a written answer. <br />4.2.4 Assemble your completed work into a package that can be readily reviewed (An index is <br />desirable). <br />4.2.5 Include the signed and completed RFP Form in your proposal submittal. <br />4.3 PROJECT APPROACH: <br />Detail your fir m s approach to the project. Include details of tasks that will be completed for <br />each phase of the project. Include specific information which will allow the evaluation <br />committee to fully understand how the project will be conducted. <br />4.3.2 Describe the administrative approach and project management techniques which will be <br />employed during the contract to ensure the coordination and timeliness of the work. <br />4.4 REFERENCES /PORTFOLIO: <br />Provide three re erences, including contact name and a current telephone number. <br />4.4.2 Provide a portfolio of two to three samples of reports on similar types of projects. <br />4.5 PROPOSER'S PERSONNEL: <br />Provide an organiza ion chart of individuals performing services <br />4.5.2 Provide resumes of key personnel to be assigned to work on the program. Provide descriptions <br />their relevant experience on previous (preferably similar) programs and projects. Provide a <br />statement that the key personnel's duties will not be delegated. In the event of award of a <br />c -ontract and that key personnel assigned become not avail-able-for the specific work, contractor <br />must provide replacements with comparable backgrounds and experience subject to DNR <br />approval and acceptance. Failure to provide adequate replacement personnel may result in <br />contract termination. <br />4.6 PROJECT SCHEDULE: <br />ive a propose sc a ule including the key milestones of the project and the proposed dates for <br />completion of each milestone. Assume the contract start date listed in the Schedule of Activities <br />section 1.0 of the Request for Proposal. <br />4.7 PRICE <br />Give both a lump sum and per hour rate price to complete the project, including all labor, materials and <br />travel to complete research, meetings with the employee leadership group and produce reports as <br />specified in section 3.0 <br />
