Appendix 3 <br /> Natural Disaster Damage Assessment Report <br /> I 'CEB <br /> A When requested by the SEB chat r_.nn, prepare report within time specified by SEB chair—man. Consult with other board <br /> members and indicate CEB concurrence in report. <br /> I Reproduce for for Damage Aseesssent Report locally as needed. <br /> C Use tern "fern" or "farmers" to cover farms, ranches, farmers, ranchers, or aquaculture operations. <br /> D Report losses to aquaculture operations separately, completing only items 1 through 9 and other applicable it—=3 of <br /> the form. <br /> • CGmplete specific items as follows: <br /> 1 Item 3. Enter: <br /> a Total nuMber of farmers in county. <br /> b Number of farmers who sustained losses as a result of natural disaster, based on a general survey including dis- <br /> cussions vith knowledgeable persons. <br /> 2 Item L. <br /> • <br /> a Indicate: ' <br /> (1) Type of natural disaster. EXAMPLES: Drought, flood, windstorm, excessive rainfall, hailstorm, blizzard, <br /> early freeze or frost, hurricane, or tornado. If a mayor A=ML OR POULTRY DISE&,SE outbreak occurs <br /> immediately following a natural disaster, state how abnormal weather may have contributed to spread of <br /> disease to epidemic stages. <br /> (2) Actual date or dates on which natural disaster occurred. Enter for use in establishing incident period. <br /> EXAMPLES: <br /> Hailstorm — August 15, 1975 <br /> Drought — Payy 1 through July 15, 1975 <br /> b Where only tart of county is involved. Attach map delineating area affected. <br /> 3 Items 5 through 9. List all principal commercial crops, including pastures and timber in the order of their <br /> importance to the country's agricultural economy. <br /> L Items 5 throe h 1L. <br /> a Craps and Livestock. Use price information furnished by the 5i3 chairmen in estimating dollar losses of crops <br /> (including pastures and timber) and livestock. - <br /> b Aquaculture Operations. Use information furnished by the 53 chairman in estimating unit and ,lntlar lasses of <br /> aquaculture operations. <br /> 3. Items 15 through 20. Use the value of farm buildings and equipment as determined by the CEB in estimating dollar <br /> losses. <br /> 6 Item 21. Include damage and losses to farmland, fences, timberland, drainage outlets, irrigation systems, etc. <br /> p' Submit written report,to SEB chairman with copies to: <br /> 1 Appropriate county government representatives. <br /> 2 ZHB members. <br /> 2 MB <br /> A Edit each county report as necessary in cooperation with FmRA, SRS, and other board members as appropriate and indi- <br /> cate SEB concurrence. <br /> D Lend each report by tail within two work days after receiving report for CEB to ASCS Regional Director, A iy 01f: <br /> emergency Preparedness Division with copies to: <br /> 1 Governor's emergency coordinator and State department of agriculture. <br /> 2 SEB members. • <br /> 3 I11AA regional director upon request. <br /> it National Weather Service, BOAR State representative.
