Prospective contractors should address their capability to comply with the above schedule. If <br />it is deemed that the above critical dates should be revised, prospective contractors should <br />offer alternative schedules describing the logic and reasons for the alternative. <br />3) Conflicts of Interest Statement addressing whether or not any potential conflict of interest <br />exists between this project and other past or on-going projects, including any projects <br />currently being conducted for the Program. <br />4) Detailed cost not to exceed proposal to complete the project, separated into protocol <br />development and protocol implementation. The proposal should identify costs and hours <br />allocated for each task in the scope of work and the total cost for the study. Hourly rates and <br />reimbursable expenses for the proposing firm/individual and any sub-contractors must be <br />attached to the detailed price proposal. The contract will be awarded on a Cost Not to <br />Exceed basis. The initial contract will be for protocol development. Governance Committee <br />approval is needed before the contractor is authorized to begin pratocol implementation. <br />5) List of relevant project experience within the past five (5) years, including name, location <br />and brief description of the projects; name, address and phone number of the contracting <br />officer for the client; and identification of key participants and their tasks on previous <br />projects who would also be working an this study. <br />6) Resumes of key participants and subcontractors proposed for this study. The resumes should <br />address experience on projects similar to this stage change study. Types of expertise that <br />may be appropriate include familiarity with whooping crane biology, aerial survey <br />techniques, the central Platte River, and data collection and reporting. <br />7} Description of Insurance shall be provided with the proposal. Proof of insurance will be <br />required before a contract is issued. Minimum insurance requirements include $1,000,000 <br />general liability per occurrence. To the extent authorized by law, the Contractor shall <br />indemnify, save, and hold harmless the Nebraska Community Foundation; the states of <br />Colorado, Wyoming, and Nebraska; the Department of the Interior; members of the <br />Governance Committee; and the Program Executive Director's Office, their employees, <br />employers, and agents; against any and all claims, damages, liability, and court awards <br />including costs, expenses, and attorney fees incurred as a result of any act or omission by the <br />contractor or its employees, agents, subcontractors, or assignees pursuant to the terms of this <br />proj ect. <br />8) A pre-bid meeting of interested parties will be held to address questions associated with this <br />Request for Proposals at a time and location that will be set by the Program Executive <br />Director's Office. <br />Please submit one electronic copy of your proposal in PDF format by February 4, 2008 to <br />Chad Smith at smithc@headwaterscorp.com. <br />Terms and Conditions: The selected contractor will be retained by: <br />Nebraska Community Foundation <br />650 J Street, Suite 305 <br />2
