3) A pre-bid meeting. A pre-bid meeting of interested parties will be held to address
questions associated with this Request for Proposals at a time and location that will be set by
the Executive Director of the Program.
4) Detailed cost of the annual costs for each policy. The proposal should identify annual
cost for each policy. In addition, a detailed description of the coverages, exclusions, and
specific terms and conditions under each policy shall be included. If more than one limit of
liability is proposed, then the costs should be provided for each policy and each level of
protection. Governance Committee approval is needed for implementation of this contract.
5) References. Names, addresses, and phone numbers of at least three references.

Governance Committee reserves the right to accept any proposal that, in its judgment, is the best
proposal and to waive any irregularities in any proposal.
