i? - `x <br />PLATTE RIVER RECOVERY IMPLEMENTATION PROGRAM <br />REQUEST FOR PROPOSALS <br />SUBJECT: Insurance Procurement <br />REQUEST DATE: November 1, 2007 <br />CLOSING DATE: November 30, 2007 <br />POINT OF CONTACT: Dr. Jerry Kenny <br />Executive Director <br />Platte River Recovery Implementation Program <br />3710 Central Avenue, Suite E <br />Kearney, NE 68847 <br />(308) 237-5728 <br />kennyj(a?headwaterscorp.com <br />RECITALS <br />The Governance Committee of the Platte River Recovery Implementation Program (Program) <br />submits this Request for Proposals (RFP) to solicit proposals to obtain insurance coverage as <br />outlined herein for the Governance Committee representatives and subcommittee representatives <br />for certain actions that will be undertaken through Program implementation. The successful <br />policy should have a limit of at least $10 million per occurrence and be by an "A-rated" insurer. <br />The attached description of the Program should be considered before developing the proposal. <br />In responding to this RFP, the Governance Committee requests the following information: <br />1) A statement of qualifications. At a minimum this should include a description of the <br />company, including, the name, address and phone number of the contracting officer and any <br />other important personnel, the number of years the company has been in business, and types <br />of insurance provided to other entities. <br />2) Scope of Coverage/Description of the Policy. Prospective contractors should provide a <br />detailed description of the coverage they are proposing to provide. At minimum the scope of <br />coverage should include coverage for the Governance Committee representatives and <br />subcommittee representatives (including alternates) and the organizations that employ the <br />representatives, for all actions that will be undertaken through Program implementation, <br />including the intentional release of a pulse flow to the Platte River. The insurance should <br />cover, and provide separate policies for at a minimum, General Liability, Property Insurance, <br />Bodily Injury, and Management Liability. Each policy should include coverage of $10 <br />million per occurrence, but any higher coverage may also be included in the proposal. <br />Several of the organizations represented on the Governance Committee (CNPPID, NPPD, <br />others) will have to be listed as co-insured. Coverage should apply regardless of any <br />limitation of liability otherwise available to the insured parties (e.g. sovereign immunity or <br />government tort claims acts). This policy will serve as primary insurance for CNPPID and <br />NPPD related to Program pulse flows. The proposal should be clear on who would be the <br />insured, and on any specific conditions that would be part of the policy.
