Resumes of key project participants and subcontractors proposed for this project. <br />Provide a table showing the tasks, responsibilities, and the minimum number of hours <br />each key participant is committing to this project. <br />4. Equipment list sufficient to complete study. The contractor will be required to have <br />available for use Microsoft Access 2003 or 2007 for data entry. <br />5. Conflicts of interest statement addressing whether or not any potential conflict of <br />interest exists between this project and other past or on-going projects, including any <br />projects currently being conducted for any party to the Program. <br />6. Scope of work for completing this project, including the technical approach to <br />completing the work and a description of project management. <br />7. Description of Insurance shall be provided with the proposal. Proof of insurance will <br />be required before a contract is issued. Minimum insurance requirements will include <br />$1,000,000 general liability per occurrence. If aircraft are to be used the insurance should <br />include coverage for both aerial and ground surveys and the same limit of coverage for <br />passengers in aircraft (no sub-limits). To the extent authorized by law, the contractor <br />shall indemnify, save, and hold harmless the Nebraska Community Foundation, the states <br />of Colorado, Wyoming, and Nebraska, the Department of the Interiar, members of the <br />Governance Committee, and the Executive Director's Office, their employees, <br />employers, and agents, against any and all claims, damages, liability, and court awards <br />including costs, expenses, and attorney fees incurred as a result of any act or omission by <br />the contractor or its employees, agents, subcontractors, or assignees pursuant to the terms <br />of this project. <br />Detailed cost not to exceed proposal to complete the project. The bid should identify <br />hours and costs by labor category for each task, a grand total, and rate sheet for the <br />project staff identifying hourly billing rates and institutional overhead costs. Other direct <br />costs such as reproduction costs, supplies, travel, etc. should be broken out by unit cost, <br />quantity, and amount ($).
