SUBNIISSION REQUIl2EMENTS 306 All interested parties having experience providing the services listed in this RFQ are requested to 307 submit a proposal. 308 309 Instructions for Submitting Proposals 310 One electronic copy of your proposal must be submitted in PDF format to Jason Farnsworth at 311 farnsworthina,headwaterscorp.com no later than S: 00 p.m. Central Time on January 30, 2009. 312 Maximum allowable PDF size is 8MB. A proposal is late if received any time after 5:00 p.m. 313 Central Time and will not be eligible for consideration. 314 315 Questions regarding the information contaimed in this RFQ should be submitted bv e-mail 316 o_nlv to Jason Farnsworth ( arnsworthKa),headrvaterscorp.com). A list of compiled contractor 317 questions and responses will be maintained on the Program web site 318 (www.PlatteRiverProsram.orQ) in the same location as this RFQ solicitation. A pre-proposal 319 meeting of interested parties will be held on Tuesday, January 6, 2009 at the Headwaters 320 Corporation/Program conference center (Suite 5, Central Business Station Center, 3710 Central 321 Avenue) in Kearney, Nebraska from 1:30-2:30 p.m. Central Time to address questions associated 322 with this RFQ. A conference call line will be available; please visit the Program web site for 323 additional conference line details. 324 325 Proposal Content 326 Proposals should conform and respond to the following general topics: 327 328 1) Execufive summary that presents brief firm overview and condenses and highlights the 329 contents of the proposal in such a way as to provide a broad understanding of the contractor's 330 qualifications and technical proposal. 331 332 2) Management approach that documents how the contractor would organize and manage the 333 project and provides the contractor's project team (including subcontractors), team 334 organization and Program Staffresponsibilities. The management approach should also 335 include previous experience similar to the Program database project and include the 336 involvemendrole of the proposed team in those projects. 337 338 3) Technical approach that demonstrates the contractor's understanding of Program database 339 system needs and functionality and outlines tasks necessary to accomplish the scope of work 340 outlined in Section III of this RFQ. 341 342 4) Schedule for completing the tasks identified in the technical approach. Due to the need to 343 collect and manage Program data, this project is time-sensitive. Proposed schedules should 344 be as aggressive as possible without compromising the ability to produce a quality product. 345 346 5) Conflict of interest statement addressing whether or not any potential conflict of interest 347 exists between this project and other past or on-going projects, including any projects 348 currently being conducted for the Program.
