A pre-proposal meeting of interested <br />321 parties will be held on Tuesday, January 20, 2009 at the Headwaters Corporation/Program <br />322 conference center (Suite 5, Central Business Station Center, 3710 Central Avenue) in Kearney, <br />323 Nebraska from 1:30-2:30 p.m. Central Time to address questions associated with this RFP. A <br />324 conference call line will be available; please visit the Program web site at <br />325 www.PlatteRiverPro ram.org for more details. <br />326 <br />327 Proposal Content <br />328 In responding to this RFP, the Governance Committee requests a proposal that includes the <br />329 following information: <br />330 <br />331 1) Project understanding and approach to accomplishing the work outlined in the project <br />332 description. <br />333 <br />334 2) Scope of work and cost proposal for completing this project. The proposal should identify <br />335 costs and hours allocated for each task in the scope of work and the total cost for the project. <br />336 Hourly rates and reimbursable expenses for the proposing firm/individual and any sub- <br />337 contractors must be attached to the price proposal. <br />338 <br />339 3) List of relevant project experience within the last five years, including name, location, and <br />340 brief description of the proj ects; name, address, and phone number of the contracting officer <br />341 for the client; and identification of key project participants and their responsibilities. <br />342 <br />343 4) Schedule for completing each task in the scope. Due to the need to collect and manage <br />344 Program data, this project is time-sensitive. Proposed schedules should be as aggressive as <br />345 possible without compromising the ability to produce a quality product. <br />346 <br />347 5) Resumes of all parties associated with the proposal, including relevant project experience. <br />348 <br />349 6) Conflict of Interest statement addressing whether or not any potential conflict of interest <br />350 exists between this project and other past or on-going projects, including any projects <br />351 currently being conducted for the Program. <br />352 <br />Page 8 of 10
