<br />Water Supply Reserve Account - Grant Application Form <br />Form Revised May 2007 <br /> <br />Task 5 State Engineer Dam Safety Permit Submittal Documents <br />The project team lead by Applegate Group will prepare all necessary submittal documents to apply for a dam safety <br />permit through the Division of Water Resources. Mike Applegate will provide a senior technical advisory role to <br />the engineering team. J. A. Cesare and Associates lead by Joe Cesare will provide geotechnical documents and <br />representation. This task will include all technical documents, project representation to the State, and negotiations <br />regarding engineering design issues that may arise. The documents included are a Hazard Classification Report; <br />Hydrology Study; Geotechnical Report; Engineering Report documenting all work done in the final design; Water <br />Diversion Control Plan; Instrumentation Plan; Monitoring Plan; Emergency Preparedness Plan; Construction Plans; <br />and Construction Specifications. An application fee to the State of$3,OOO is included in the fee. <br />Deliverables for this task will include: <br />I. A complete permit application to the State of Colorado meeting current dam safety criteria and guidelines <br />stamped and sealed by a registered professional engineer with a Colorado license. <br />2. Representation of the project during State review, revisions of documents required under the review, and <br />negotiations with the State on engineering issues related to the submittal materials. Client reporting on progress <br />during the 6 month initial review process to the final decision on issuance of the permit. <br />The total fee for Task 5 is $98,000 <br /> <br />Task 6 Bid Documents and eontract Agreements and Bid Assistance Services <br />The project team lead by Applegate Group will prepare draft bid documents and Construction Contract agreements <br />using EJCDC templates approved for projects of this scope and complexity. Bennett Raley will provide legal <br />review of all documents. These documents will be subject to final review and acceptance by the ownership group <br />for the District 64 Reservoir Company. Bid Documents will include a detailed construction bid sheet detailing <br />quantities where appropriate, and instructions for bidders. The Contract Documents will include General <br />Conditions, Special Provisions, and sample submittal documents that will be used during construction. Other <br />services provided will include Contractor prequalification prior to bidding, preparation and publication of an <br />Advertisement for Bid, conducting a pre-bid meeting for potential bidders to answer questions, preparation of <br />written responses to bidder questions, conducting a bid opening for the Owner Group, review and checking of the <br />received bids, and making a recommendation to the Owner Group on the apparent low responsive bid for the <br />project. Once the Owner Group makes a decision on selecting a Contractor, Applegate Group will prepare <br />necessary documents such as Notice of Award, Notice to Proceed, and review of Contractor submittals such as <br />bonding and insurance. <br /> <br />11 <br />
