<br />Water Supply Reserve Account - Grant Application Form <br />Form Revised May 2007 <br /> <br />Deliverables for this task will include: <br />1. A complete set of Bid Documents and Contract Agreements prepared under EJCDC format that are suitable for <br />guiding construction of a project of this complexity. All documents are subject to review by legal counsel and <br />acceptance of the Owner Group. <br />2. A summary document of all material used in receiving bids for the project. <br />3. Technical assistance through the Bid Process. <br />The total fee for Task 6 is $43,000 <br /> <br />Phase II <br /> <br />Task 7 - Construction Observation Services and Project Documentation <br />The project team lead by District 64 and their consultants will provide an experienced Project Management team of <br />engineers and technicians that will provide on-site services during construction of the project. These services <br />include observation of the constructed project at critical points, providing quality control testing of materials and <br />work, providing Project Management services with regular reporting of construction progress to the State of <br />Colorado and the Owner group on a monthly basis, and providing final documentation to the State of Colorado at <br />the point of substantial completion to obtain necessary approvals for operation of the project. All documents <br />pertinent to the Owner Group managing and operating the project will be compiled into an Owner's Manual. <br />Follow-up services for Dam Safety monitoring and reporting will be provided as required and detailed in the Dam <br />Safety regulations. This phase of work is somewhat speculative because of the unknowns that are involved in final <br />decisions resulting from previous tasks. The project duration is estimated. The involvement of the Owners Group <br />during construction is also unknown. This task has an estimated fee, but will be subject to review and revision <br />based on outcomes of other portions of the project. <br />Deliverables for this task will include: <br />1. Engineering support services for the duration of construction including reporting to involved parties that meet <br />Dam Safety rules and regulations. <br />2. A final report to the State Engineer's Office on the constructed project meeting submittal requirements outlined <br />in the rules and regulations for Dam Safety, current edition. <br />3. An Owner's Manual of the completed project that incorporates all product information for equipment used. Any <br />SCADA, telemetry or other control equipment will be documented in the process including written operating <br />instructions. Field training is not included in this scope. <br />The total fee for Task 7 is $1,750,000 <br /> <br />12 <br />
