Set up and use of a water pump outside the approved affected area to <br />divert water. This pump was set up at this location as a result of the town <br />of Alma's request. The pump was being set by our crew inside the permit <br />when a representative from the town demanded that it be moved. As a <br />result the pump was set approximately 15 feet off of the permit boundary. <br />Mike Itel the Water Commissioner verbally approved this location as a <br />temporary water source. The pump was moved back to within the permit <br />area on June 9` On June 13 the Columbia Ditch was opened and we <br />discontinued the use of the pump at that time. <br />2. We have not properly installed the approved process water treatment <br />Ponds in accordance with the approved application. Furthermore, ponds <br />that have been installed are malfunctioning by allowing leakage and loss <br />of containment of the process water. We have always used a three -pond <br />system to settle out our fines from the wash plant. The first two ponds are <br />long and narrow so that an excavator can clean them daily. The third pond <br />is much larger in order to settle out the very fine particles. This pond <br />developed a leak as we unknowingly built it over a deposit of clean <br />cobble. The pond was sealed with crushed fines on June 9 <br />3. We have failed to install storm water control ponds 4, 5, & 6 and their <br />associated collection ditches as required by the approved surface water <br />managementplan. The collection ditch was rebuilt on July 5 and pond <br />#4 was increased to the designed size on August 4 As a result of your <br />follow up inspection we are going to wait to construct ponds 5 and 6 <br />because we have not yet impacted those areas. <br />p. 2 <br />
