Reclamation Permit Conversion Application Package – Second Incompleteness Notice
On July 1, 2011, the Division of Reclamation, Mining and Safety (Division) received the revised
110 to 112 Construction Materials Reclamation Permit Conversion Application (CN -01) package
for the Fairbanks Pit, File No. M- 2003 -058. Preliminary review of the information received
determined the following items must be received before the Division can consider the application
as being submitted and technical review can begin:
Application Form
1. The Division requires the Operator name remain consistent and matches the exact name
registered with the Secretary of State's office ( "Glenn E. Sessions & Sons, Inc.") throughout
the application. Please provide the Division with a revised and original (signatures in blue ink)
copy of the application form. A copy of the Colorado Secretary of State summary page for
Glenn E. Sessions & Sons, Inc. and an edited copy of the application form are attached for
reference.
2. The existing permitted acreage for the Fairbanks Pit is 7.040. Please revise Item #3 on Page
1 of the application form to indicate 7.040 permitted acres with a change in acreage of
12.02 (5.623 acres – Fairbanks II Pit and 6.397 acres - Pre -Law area) for a total permit
acreage of 19.06.
3. The Division did not receive an orignial copy of the revised conversion application form for
the Fairbanks Pit. Please submit one original and one copy of the revised application form
and any other revised exhibits.
Exhibit C - Pre - mining and Mining Plan Map(s) of Affected Lands (Rule 6.4.3)
The version of Exhibit C – Pre - mining and Mining Plan Map submitted by the Applicant on July
1, 2011 was an improvement over the previous map, however the map still does not comply
with all the requirements of Rule 6.2 and 6.4.3.
