Pineda <br />Director <br />The Division of Reclamation, Mining and Safety (Division) has reviewed the content of the Mountain <br />Park Concrete, Inc. 112c permit conversion application for the Parshall Pit, File No. M-1998-034, and <br />submits the following comments. The Division is required to make an approval or denial decision no <br />later than August 23, 2010 therefore; a response to the following adequacy review concerns should be <br />submitted to the Division as soon as possible. <br />Application Form <br />1. The Division requires the Operator's name remain consistent and matches the exact name registered <br />with the Secretary of State's office (Mountain Park Concrete, Inc.) throughout the application. Please <br />provide the Division with a revised and original (signatures in blue ink) copy of page 8 of the <br />application form. A copy of the summary page from the Colorado Secretary of State for Mountain <br />Park Concrete, Inc. is attached for reference. <br />Application Comments <br />2. The Division received comments from the Colorado Division of Wildlife. The letter is attached for <br />review. Please address the comments noted in the letter and make any changes to the application <br />as necessary. <br />6.4.1 Exhibit A - Legal Description <br />3. The Legal Description must match the survey description exactly.
