Pirieda <br />Director <br />The Division has identified several other comments and questions that must be addressed prior to <br />the Division's decision due date on June 3, 2010. Please be advised that if you are unable to <br />satisfactorily address any concerns identified in this review before the decision date, it will <br />be your responsibility to request an extension of the review period. If there are outstanding <br />issues that have not been adequately addressed prior to the end of the review period, and <br />no extension has been requested, the Division will deny this application. <br />6.4.3 Exhibit C - Pre-mining and Mining Plan Map(s) of Affected Lands <br />1. The revised Mining Plan Map did not list all adjoining surface owners of record. Please <br />include this information on the Mining Plan Map. <br />2. The topsoil and product stockpiles are shown on the Mining Plan Map; however, they do not <br />have labels. Please label the topsoil stockpiles and the product stockpiles, so the Division can <br />easily identify them in the field. <br />3. The Applicant has indicated a note describing the typical vegetation of the site has been <br />included on the Mining Plan Map. The Division could not locate the note on the map. Please <br />revise the Mining Plan Map to include the type of present vegetation covering the affected <br />lands. <br />4. The Applicant has depicted the power lines within 200 feet of the affected land on the <br />Mining Plan Map. As required by Rule 6.4.3(g), please list the owner's name for each of the <br />power lines. <br />5. The Mining Plan Map has labeled the permit boundary as the `survey boundary'. To prevent <br />confusion in the future, please change `survey boundary' to `permit boundary' to be <br /> <br />Office of Office of <br />Mined Land Reclamation Denver • Grand Junction • Durango Active and Inactive Mines
