The Division has identified several other comments and questions that must be addressed prior to
the Division's decision due date on June 3, 2010. Please be advised that if you are unable to
satisfactorily address any concerns identified in this review before the decision date, it will
be your responsibility to request an extension of the review period. If there are outstanding
issues that have not been adequately addressed prior to the end of the review period, and
no extension has been requested, the Division will deny this application.
6.4.3 Exhibit C - Pre-mining and Mining Plan Map(s) of Affected Lands
1. The revised Mining Plan Map did not list all adjoining surface owners of record. Please
include this information on the Mining Plan Map.
2. The topsoil and product stockpiles are shown on the Mining Plan Map; however, they do not
have labels. Please label the topsoil stockpiles and the product stockpiles, so the Division can
easily identify them in the field.
3. The Applicant has indicated a note describing the typical vegetation of the site has been
included on the Mining Plan Map. The Division could not locate the note on the map. Please
revise the Mining Plan Map to include the type of present vegetation covering the affected
lands.
4. The Applicant has depicted the power lines within 200 feet of the affected land on the
Mining Plan Map. As required by Rule 6.4.3(g), please list the owner's name for each of the
power lines.
5. The Mining Plan Map has labeled the permit boundary as the `survey boundary'. To prevent
confusion in the future, please change `survey boundary' to `permit boundary' to be
