Box 147 <br />Fairplay, CO 80440 <br />Re: Park County Road & Bridge; Ansley Pit #2; Park County; Permit No. M-1995-027- <br />Conversion Application CN-01; Adequacy Review No. 2 <br />Dear Mr. Keith: <br />The Division of Reclamation, Mining, and Safety (Division) has review the additional information <br />that you provided to this office in regards to the Ansley Pit #2 Conversion Application, Permit No. <br />M-1995-027. Listed below is the additional information we require in order to process the <br />application. <br />General Operation Information <br />Part 5. Applicant must indicate on the Permit Application Form whether the 500 refers to lbs per <br />year or tons per year. A copy of the page is enclosed. Please indicate by circling the appropriate <br />entry on the page and return the page to the Division. <br />Rule 6.4.3 Exhibit C - Pre-Mining and Mining Plan Maps of Affected Lands <br />The following items are missing from the maps required in this exhibit: <br />Rule 6.4.3(a). All adjoining surface owners of record. An example of Exhibit C showing this <br />information is enclosed. If desired, you may show the adjoining surface owners on the <br />enclosed 8x11 Land Survey Plat and return it to the Division. <br />Rule 6.4.3(b). The names and locations of all creeks, roads, buildings, oil and gas wells and lines, <br />and power and communication lines on the area of affected land and within 200 feet of all <br />boundaries of the area. An example of a correctly completed Pre-Mining Plan Map is enclosed <br />for your reference. <br />Rule 6.4.3(g). The owner's name, type of structures, and location of all significant, valuable, and <br />permanent man-made structures contained on the area of affected land and within 200 feet of <br />the affected land. This includes public roads, power lines, and buildings. As an example, the <br />Allen Pit (DRMS file No. M-2005-080) immediately to the southeast of the site appears <br />to possess structures within 200 feet of the Ansley Pit #2 site. <br />Rule 6.4.17 Exhibit O - Proof of Mailing of Notices to Board of County Commissioners and Soil <br />Conservation District <br />The Division is unable to find in the application materials proof that notice was sent, pursuant to Rule <br />1.6.2(1)(a)(ii), to the Board of County Commissioners and to the local soil conservation district. <br />Office of Office of <br />Mined Land Reclamation Denver • Grand Junction • Durango Active and Inactive Mines
