M-1995-027 <br />22 April 2010 <br />Pg. 2 <br />The Teller-Park Soil Conservation District has jurisdiction in this area. Pursuant to Rule <br />1.6.2(1)(a)(ii), proof of notice must be in the form of a return receipt of a Certified mailing or a date- <br />stamped copy of the notice acknowledging receipt by the appropriate local Board. <br />For your reference, the blank forms have been enclosed, along with a copy of the notice provided to <br />the Teller-Park SCD for the original application back in 1995 to illustrate what constitutes valid proof <br />of mailing. <br />Rule 6.4.19 Exhibit S - Permanent Man-made Structures <br />Permanent man-made structures within 200 feet of the site include the building(s) on the property <br />south of the mine (The Allen Pit, DRMS file No. M-2008-080), power limes, and Colorado State <br />Highway 9. As required by Rule 6.4.19, please provide either (a) notarized agreements, (b) <br />engineering evaluations, or, if the structure is a utility, (c) a notarized letter. <br />Decision Date <br />The Division has extended the decision date on this application to 28 May 2010.
