Box 212 <br />Antonito, CO 81120 <br />RE: Incomplete Appliga"tion for Conversion a 112c Permit (CN-01), <br />Robins Pit No. 1, File No. M-2002-014. <br />Dear Mr. Johnson and Mr. Robins: <br />On June 29, 2009, the Division received the above referenced conversion application. <br />Preliminary review of the information submitted indicates that the application is incomplete for the <br />purposes of filing. The following items must be submitted prior to the Division considering the <br />application filed and commencement of the 90-day decision-making time period, per Rule <br />1.4.5(2). <br />1. The Division received two copies of the application and not an original with original <br />notarized signatures. The Division requires one original with original notarized signatures and <br />one complete copy of the application and all exhibits. Please ensure that the Division receives <br />one original with original notarized signatures and one complete copy of the application. <br />2. The Division requires the Operator company name listed throughout this application match <br />the company name listed with the State of Colorado's Secretary of State Office (i.e., "Robins <br />Construction, LLLP") rather than the company name represented by the Notary Public on page <br />eight of the application form ("Robins Construction L"), and the company name represented on <br />page one ("Robins Construction LLLP.") and page three ("Robins Construction Company LLLP.") <br />of the application form. Please ensure that the appropriate company name is utilized throughout <br />the application. A new application form has been enclosed for your use. Copy of the application <br />forms and the Construction Materials Act and Rules are also available at the Division's web site <br />at www.mininq.state.co.us. <br />3. Several lines of the application form were left blank and did not provide the required <br />information. Specifically, lines 1.1, 5.1, 5.3, and 14 were not completed.
