The report shall <br />include, but not be limited to, text, discussion and maps lvhich address: <br />PAP, page 2.04-82. An other agreed on date is March 15 of each year, see page 2.04-82 of the Permit <br />Application Package (PAP). <br />The Annual Reclamation Report (ARR) for 2004 is incompliance with this rule as agreed upon in the <br />PAP. The cover letter was dated February 10, 2005, and it was received at the Division of Minerals and <br />Geology (Division) on February 23, 2005 (an advance copy was handed to the Inspector on February 16, <br />2005 during a Division inspection. <br />Rule 2.04. l3(1)(a) The name and address of the permittee and permit number,' <br />The report is in compliance with this rule.
