Long <br />Dwision Dnec mr <br />Dear Mr. Million: <br />On September 6, 1994, the Division of Minerals and Geology received your <br />112 Reclamation Permit application for the Spring Ck Gravel Pit, File <br />No. M-94-097. Preliminary review determined that the following items must be <br />submitted before review of your application can begin: <br />Pre-mining, Mine Plan, and Reclamation Plan Macs: All maps, except the <br />index map, must conform to the following criteria: <br />General Requirements (these are the requirements for each type of map <br />required): <br />1. Show the name of the applicant; <br />2. Must be prepared and signed by a registered land surveyor, <br />professional engineer, or other qualified person; <br />3. Give the date prepared; <br />4. identify and outline the area which corresponds with the <br />application; <br />5. Be made to scale not larger than 1" = 100' and not smaller than 1" _ <br />660', which shall be noted on each map; maps, not including the <br />accurate map of the affected lands designated in <br />Section 34-32-112(4), C.R.S. 1973, as amended, may be prepared at <br />other scales as long as the scale is appropriate so as to clearly <br />show those elements that are being delineated; and <br />6. Must have a title and legend (explanation) indicating the items <br />delineated on that particular map. <br />SPECIFIC REQUIREMENTS: <br />EXHIBIT C - Pre-mining and Mine Plan Macs: <br />One or more maps may be necessary to legibly portray the following <br />information: <br />1. All immediately adjacent surface owners of record; <br />2. The name and location of all creeks, roads, buildings, oil and gas <br />wells and lines, and power and communication lines on the area of <br />affected land and within two hundred (200) feet of all boundaries of <br />such area; <br />
