- <br />3. The existing topography of the area with contour lines of sufficient <br />detail to portray the direction and rate of slope of the affected <br />land; <br />4. The total area to be involved in the operation, including the area <br />to be mined and the area of affected lands; <br />5. The type of present vegetation covering the affected lands; <br />6. Show the name and location of all permanent man-made structures <br />contained on the area of affected land and within two hundred <br />(200) feet of the affected land. <br />EXHIBIT F - Reclamation Plan Mao: <br />1. The expected physical appearance of the area of the affected land, <br />correlated to the proposed mining and reclamation timetables. The <br />map must show proposed topography of the area with contour lines of <br />sufficient detail to portray the direction and rate of slope of all <br />reclaimed lands; and <br />2. Portrayal of the proposed final land use for each portion of the <br />affected lands. <br />EXHIBIT O - Proof of Mailing of Notices to the County Commissioners and <br />Soil Conservation District: <br />You must submit proof of actual delivery or proof of mailing by <br />Certified Mail, return receipt requested, the form contained in the <br />application package called "Notice of Filing Application" to the <br />Montrose County Board of County Commissioners and, if the mining <br />operation is within the boundaries of a Soil Conservation District, <br />to the Board of Supervisors of the Soil Conservation District. <br />EXHIBITS R - Proof of Filin4 with County Clerk and Recorder <br />You must submit an affidavit or receipt indicating the date on which <br />the application was placed with the local County Clerk and Recorder. <br />In the preliminary review of the documents submitted it was noted that the <br />copies of the exhibits submitted were not complete and identical to the <br />exhibits submitted with the original application. All five sets of exhibits <br />must be the same. Also, the application and exhibits submitted to the <br />Montrose County Clerk and Recorder must be identical to the documents <br />submitted to the Division. Please submit the additional copies as required to <br />the Division to complete the exhibit packages. <br />Failure to submit the items listed above by the initial decision date of <br />December 5, 1994, may result in a decision to deny your application. You may <br />amend your application by submitting the requested items. In accordance with <br />Rule 1.8, a new decision date will be established 90 days £rom the date of <br />receipt of all requested items. If you have already initiated publication, <br />you will need to republish notice which specifies the final date for receiving <br />comments is the 20th day after the fourth publication or the next regular <br />business day. <br />This letter shall not be construed to mean that there are no other technical <br />deficiencies in your application. The Division will review your application <br />to determine whether it is adequate to meet the requirements of the Act after <br />submittal of all required items. <br />
