Walther <br />Ezecutive Director <br />Michael g. Long <br />Division Director <br />DMG has completed the preliminary adequacy review of your Upland Gravel permit application. This <br />application cannot be approved until the following issues are adequately addressed. Please submit a <br />response to the following issues no later than October 3, 2001 so the decision due date of October 9, <br />2001 can be met. If you cannot respond by October 3, 2001, Please submit a written request for <br />extension of the decision date. If your response, or an extension request, are not received by October <br />3, 2001 this application may be denied. <br />ADEQUACY ISSUES <br />Exhibit B -Site Description <br />1. In addition to the Public Service power lines it appears that there are fences, water mains, a water <br />storage pond and county roads within 200 feet of the proposed permit area. Please address these <br />structures as required by Rule 6.3.2{b) and revise the maps accordingly. <br />2. If Mesa County has an easement for the 35 Road extension that lies within the proposed permit <br />area, please identify this easement. <br />Exhibit C -Mining Plan <br />3. Please specify how much water will be used annually by the operation, including dust control. <br />Identify the source for water that will be utilized until water rights are secured for the waste irrigation <br />water and storage water. <br />4. Please specify which of the proposed structures are portable and which will require permanent <br />footers or foundations. For the permanent structures, please provide dimensions and construction <br />specifications for use in calculating demolition costs. <br />5. Because of the lack of topsoil at this site and the probability for the pit floor to be a hard shale layer, <br />please commit to salvage and stockpiling of all overburden, crusher fines, screening fines, and <br />sediment pond cleanings to be replaced 4 inches thick over the site at reclamation as a growth <br />medium. <br />
