The operator intends to be bonded for a maximum disturbance of 65 acres. This is anticipated to <br />occur during Phase 3 of the mining plan. Exhibit L -Reclamation Cost table has been revised to <br />reflect the 65 acres of disturbance. <br />4. The operator will verify and submit to the Division the average width and depths of the slurry wall <br />to be installed prior to mineral extraction. If the slurry wall average depth will be greater than 48 <br />feet (45' deposit + 3' keyed into bedrock) and 2) if the slurry wall average depth will be less than <br />3.8 feet, the operator will submit a revised reclamation cost and/or stability analysis as deemed <br />necessary by the Division. <br />8. The operator will submit within 60 days of slurry wall completion a final ("as built) construction <br />report summarizing testing results for the tests listed in Table 1 -Materials Quality Control <br />Program of the Technical Specifications for the Milliken Site Slurry Wall Construction. <br />EXHIBIT G -WATER INFORMATION <br />14. The applicant will to monitor the groundwater levels near the cottonwoods before mining begins <br />as follows: monthly January through May and September through December and twice a month in <br />June through August for at least the first year. This data will be correlated with the flows in the <br />river [o establish a relationship between the groundwater and the river stages.
