Dear Everett and Florence Jackson,
Listed below are adequacy review comments on the Canyon Rim Stone Quarry 110c
Reclamation Permit Application. The decision date for your application is scheduled for August
14, 2006. Therefore, I would appreciate a response to these comments from you as soon as
possible. Any public comments that we receive on your application during the interim will be
forwarded to you. Please be advised that your reclamation permit application maybe deemed
inadequate and maybe denied on August 14, 2006 unless the following adequacy review items
are addressed to the satisfaction of the Division. You can request an extension of the decision
date should you require additional time.
This application process is being carried out in parallel to the process concerning a reason to
believe a violation exists at the Canyon Rim Stone Quarry. Specifically that there was a reason
to believe that there was a failure to obtain from the board or office a reclamation permit prior to
engaging, in a new mining operation (C.R.S. 34-32.5-109(1)). As part of the Corrective Action
the Division is requesting that you apply for and receive a reclamation permit, which would
include posting of the necessary reclamation bond amount.

1. Please describe the type, extent, and level of impact mining operations that have occurred to
date at the site.
2. Please provide the approximate location of the stock tank and pipeline in relation to the
