The Division cannot approve the application until <br />the remaining comments, listed below, have been adequately addressed. Please respond as soon <br />as possible, or at least by 1:00 P.M. on June 20, 2000, so the decision due date set for June 20, <br />2000 can be met. If you cannot adequately respond by 1:00 P.M. on June 20, 2000, please <br />submit a written request for an extension of the decision due date. Please be aware, if your 2nd <br />adequacy review response, or a written request for extension of the decision due date is not <br />received, by the Division, by 1 :00 P.M. on June 20, 2000, this application may be denied. <br />ADEQUACY REVIEW COMMENTS <br />EXHIBIT C -Pre-mining and Mining Plan Maps <br />1. Response acceptable. <br />2. Please indicate if the two structures, shown as residences in the trailer park, are mobile homes <br />or permanent homes. Also please revise the Exhibit C-1 map to include the CDOT garage in <br />the list of structures. <br />3. The Division's file does not include proof that Steve Karp et al and Ute Water received the <br />public notice as required by Rule 1.6.2 111(e). Please provide proof that these entities were <br />noticed appropriately. <br />4. Response acceptable. <br />5. Response acceptable. <br />Exhibit D -Mining Plan <br />6. Please indicate, on the mine plan map, the pit scenario where the operator assumes the worst <br />case scenario for reclamation liability exists. Please include the direction of mining and the <br />location of overburden stockpiles, 10 acres of backfilled area to be reclaimed, the <br />processing/stockpile area, as well as the location of salvaged topsoil stockpiles at that time. <br />
