Lon6 <br />RE: Documented Quote, Grassy Gap Coal Mine Maintenance Project (PKA-0-1074'"ision Direclnr <br />Enclosed is a brief description, site map and bid schedule for the Grassy Gap Coal Mine <br />Maintenance Project. <br />Site Showing <br />There will not be a pre-bid meeting for this project. The Project area is located approximately <br />fifteen miles south of Hayden, Colorado, in Routt County (see attached map). <br />CONTRACTORS are encouraged to visit the site prior to bidding. Please contact the landowner <br />prior to visiting the site. Ms. Janet Binns, of the Division of Minerals and Geology (303-866- <br />3567), may provide the owner's phone number. <br />Bid Submission and Evaluations <br />Bids shall be submitted by 3:30 p.m. on August 31, 1999 by fax, mail or in person to Yvonne <br />Brannon. Telephoned bids will not be accepted. The attached bid schedule must be submitted <br />by all bidders. Award will be made to the lowest Grand Total bid. <br />Bid Award <br />The apparent low bidder must submit a certificate of insurance with the following limits: <br />Standard Worker's Compensation and Employer Liability, as required by State Statute <br />including occupational disease, covering all employees at the work site. <br />General Liability (Public Liability & Property Damage) (Minimum); <br />a. Combined single limit of $600,000 written on an occurrence basis; <br />b. Any aggregate will not be less than $1 million; <br />c. The contractor must purchase additional insurance if claims reduce the annual <br />aggregate below $600,000; <br /> <br />
