d. The State of Colorado must be named as additional insured on each <br />comprehensive general liability policy; <br />e. Certificates of insurance must be provided to the State within 10 working days <br />after receipt of Award; and, <br />f. Insurance shall include provision providing notification to the State 10 days prior <br />to cancellation of the coverage. <br />3. Automobile Liability (Minimum). The contractor must carry a minimum of $600,000 <br />combined single limit. <br />VENDOR OFFSET <br />Pursuant to C.R.S. 24-30-202.4 (as amended), the State Controller may withhold debts owed to <br />state agencies under the vendor offset intercept system for: (a) unpaid child support debt or <br />child support arrearages; (b) unpaid balance of tax, accrued interest, or other charges specified <br />in Article 22, Title 39, C.R.S.; (c) unpaid loans due to the student loan division of the <br />Department of Higher Education; (d) owed amounts required to be paid to the unemployment <br />compensation fund; and (e) other unpaid debts owing to the State or any agency thereof, the <br />amount of which is found to be owing as a result of final agency determination or reduced to <br />judgment as certified by the controller. <br />Work Description <br />The attached General Conditions for Purchase Orders and Purchase Order Terms and <br />Conditions apply to all work. <br />Maintenance work to be accomplished at the Grassy Gap Coal Mine includes construction of <br />two (2) low water crossings, repair of a sediment pond dewatering device and sealing of the <br />base of the same sediment pond. A few miscellaneous tasks will be accomplished through the <br />use of machine time. Maintenance of the Grassy Gap Mine is being accomplished by the State <br />of Colorado, Division of Minerals and Geology, using forfeited reclamation bond monies. <br />The Project is scheduled to be conducted during the week of October 11, 1999. It is anticipated <br />that the project will be accomplished in no more than four (4) consecutive working days. <br />The Grassy Gap Coal Mining operation was a permitted mining operation. The Colorado Mined <br />Land Reclamation Board has revoked the operating permit, and forfeited the reclamation bond. <br />The majority of the site was reclaimed by the coal mining company. The Division of Minerals <br />and Geology periodically performs reclamation related maintenance work at the site. <br />Accomplishing the required maintenance will help to stabilize the site by providing for control of <br />snow melt and storm related run off, and by minimizing seepage from a sediment control pond <br />located on a hillside. <br />NOTE that Mobilization and Demobilization are not included as a separate line item. <br />CONTRACTOR shall include charges for these items, if any, in the prices bid for the tasks <br />described below. No additional payment for Mobilization and Demobilization will be made. <br />
