Lon6 RE: Documented Quote, Grassy Gap Coal Mine Maintenance Project (PKA-0-1074'"ision Direclnr Enclosed is a brief description, site map and bid schedule for the Grassy Gap Coal Mine Maintenance Project. Site Showing There will not be a pre-bid meeting for this project. The Project area is located approximately fifteen miles south of Hayden, Colorado, in Routt County (see attached map). CONTRACTORS are encouraged to visit the site prior to bidding. Please contact the landowner prior to visiting the site. Ms. Janet Binns, of the Division of Minerals and Geology (303-866- 3567), may provide the owner's phone number. Bid Submission and Evaluations Bids shall be submitted by 3:30 p.m. on August 31, 1999 by fax, mail or in person to Yvonne Brannon. Telephoned bids will not be accepted. The attached bid schedule must be submitted by all bidders. Award will be made to the lowest Grand Total bid. Bid Award The apparent low bidder must submit a certificate of insurance with the following limits: Standard Worker's Compensation and Employer Liability, as required by State Statute including occupational disease, covering all employees at the work site. General Liability (Public Liability & Property Damage) (Minimum); a. Combined single limit of $600,000 written on an occurrence basis; b. Any aggregate will not be less than $1 million; c. The contractor must purchase additional insurance if claims reduce the annual aggregate below $600,000;
