iiiii iiiii <br /> <br />q-z 9 -~9 <br />DATE: May 1, 1999 <br />T0: Interested Contractors <br />FROM: Steven Renner, Division of Minerals and Geology <br />RE: Documented Ouote, Grassy Gap Coal Mine Maintenance Project (PKA-9-XXX) <br />Endosed is a brief description, site map and bid schedule for the Grassy Gap Coal Mine Maintenance Project. <br />Sits Showing <br />There will not be apre-bid meeting for this project. The Project area is located approximately frfleen miles south of <br />Hayden, Colorado, in Routt County (see attached map). <br />CONTRACTORS are encouraged to visit the site prior to bidding. Please contact the landowner prior to visiting the <br />site. Ms. Janet Binns, of the Division of Minerals and Geology (303.866-3567), may provide the owners phone <br />number. <br />Bid Submission and Evaluations <br />Bids shall be submitted by 2:30 p.m. on XXX, 1999 by fax, mail or in person to Yvonne Brannon. Telephoned bids will <br />not be accepted. The attached bid schedule must be submitted by all bidders. Award will be made to the lowest <br />Grand Total bid. <br />Bid Award <br />The apparent low bidder must submit a cert~cate of insurance with the following limits: <br />Standard Worker's Compensation and Employer Liability, as required by State Statute inducting occupational <br />disease, covering all employees at the work site. <br />2. General Liability (Public Liability & Property Damage) (Minimum); <br />a. Combined single limit of $600,000 wdHen on an occurrence basis; <br />b. Any aggregate will not be less than $1 million; <br />c. The contractor must purchase addilonal insurance'rf daims reduce the annual aggregate below <br />$600,000; <br />d. The State of Colorado must be named as additional insured on each comprehensive general liability <br />
