An appropriate reclamation plan was not submitted for the proposed downcomer pipe <br />associated with ventilation shafr #3. As .-r;th the downcomer pipes proposed for PR#07, <br />the downcomer pipe for ventilation shafr #3 should be fully grouted from surface to total <br />depth at the time of reclamation. The Minor Revision No. 216 application indicates that <br />only the downcomer connection will be sealed. Please submit a revised reclamation plan <br />for the ventilation shaft #3 downcomer pipe, which provides detail on sealing the <br />downcomer pipe with cement grout for its entire length. Reclamation should be consistent <br />with the proposed plan for the two downcomer pipes associated with PR#07. <br />2. Borehole #3 (approved with MR#214) is proposed to contain the downcomer pipe. The <br />reclamation plan for borehole #3 was approved with MR#214 application. The Division <br />approved this reclamation plan with the understanding that borehole #3 will have steel <br />casing cement grouted in place for its entire length. Please verify that borehole #3 will <br />be steel cased for its entire length, fully cement grouted in place. <br />Based on review of [he surface water hydrology and reclamation portions of the application, the <br />Division has the following additional adequacy comments. <br />3. Please revise the permit application narrative to indicate that the MSE wall at the Shafr <br />#3 wall will be permanent. <br />4. The narrative indicates the wall will be on the east side of the pad, but Figure 4 of the <br />supplemental drawings shows the MSE wall on the southwest side. Please clarify this <br />appazent discrepancy. <br />
