Reclamation Permit
On July 2Q, 2006, the Division of Minerals and Geology received your 112d-3 Designated Mining Reclamation Permit
Amendment Application package for the San Luis Project, Permit No. M-1988-112. Preliminary review of the
information received determined that the following items must be received before the Division can consider your
application as being submitted and technical review can begin:
APPLICATION FORM: Not submitted. Please complete the enclosed Designated Mining Regular (112d)
Operation Reclamation Permit Application Package.
EXHIBIT A -Leval Description (Rule 6.4.11: The legal description must identify the affected land and be
wholly adequate for field location of the property. It can be in the form of metes and bounds survey or a
description by reference to township, range and section to at least the nearest quartet quarter section. Where
applicable, the sheet address or lot numbers maybe used.
EXHIBIT P - MuniciuaGties Within Two Miles (Rule 6.4.161: Please list any municipality(s) within two
miles of the proposed mining operation and address of the general office of each municipality.
EXHIBIT O -Proof of Mailine of Notices to Counri Commissioners and Soil Conservation District (Rule
6.4.1T: Please submit proof of actual delivery or proof of mailing by Certified Mail, return receipt requested, of
Notice of the Application to the local Boazd of County Commissioners and, if the mining operation is within the
boundaries of a Soil Conservation District, to the Board of Supervisors of the Soil Conservation District.
EXHIBIT R -Proof of Filine with Counri Clerk and Recorder (Rule 6.4.181: You must submit an affidavit
or receipt indicating the date on which the application was placed with the local County Clerk and Recorder.
EXHIBIT S -Permanent Mau-Made Structures (Rule 6.4.19): You must provide information sufficient to
demonstrate that the stability of any stmctures located within two hundred (200) feet of the operation or affected
land will not be adversely affected.
EXHIBIT T - Designated Minine Operation Environmental Protection Plan (Rule 6.4.191:
The Environmental Protection Plan shall describe how the Operator/Applicant will assure compliance with the
provisions of the Act and Rules in order to protect all areas that have the potential to be affected by designated
chemicals, toxic or acid-forming materials or acid mine drainage. hi addition, the plan shall include an
Emergency Response Plan that complies with Sections 34-32-103(4.9) and 34-32-116.5(5), C.R.S. 1984, as
