Preliminary review of the information received <br />determined that the following items must be received before the Division can consider yow application as being <br />submitted and technical review can begin: <br />APPi.iCATiON FORM: Page 1, item 2: The Division requires that the site name be the same as the name on <br />the permit. Page 1, item 3: The Division requires receipt of an acreage reduction request. The Division cannot <br />release acreage through a conversion. Page 3, item 11: Needs completing. <br />EXHiRTT R -Site Descr~nfinn (gple 6~i.2hNe You must address Item (a) to the extent necessary [o <br />demonstrate compliance with the applicable performance standard requirements of Rule 3. At a minimum, you <br />must include a description of the vegetation and soil characteristics in the area of the proposed operation. The <br />local office of the Natural Resources Conservation Service (NRCS) may provide you with this information as <br />well as recommendations for Exhibit D -Reclamation Plan; <br />F.XRTRiT R -Site Descri~ lion !,Role 63.2~~~,; You must address Item (b) to the extent necessary to <br />demonstrate compliance with the applicable performance standazd requirements of Rule 3. At a minimum, you <br />must identify any permanent man-made structures within two hundred (200) feet of the affected area and the <br />owner of each structwe. Permanent man-made structwes include houses, barns, fences, above or below Bound <br />utilities, irrigation ditches, maintained or public roads, bridges, wells, pipelines, water storage impoundments, <br />railroad tracks, cemeteries, and communication antennas, etc. Each structure should be located on Exhibit E - <br />Map; <br />F.XHiRiT R -Site Description (Role 6 i2t~c11: you must address Item (c) to the extent necessary to <br />demonstrate compliance with the applicable performance standard requirements of Rule 3. At a minimum, you <br />must give a description of the water resources in the area of the proposed operation. Identify any streams, <br />springs, lakes, stock water ponds, ditches, reservoirs, and aquifers which would receive drainage directly from <br />the atI'ected area.
