Proof of publication of public notice circulated in the local newspaper is enclosed in the <br />form of a signed affidavit from the newspaper. All owners of record of surface lands, <br />easement holders, and owners of record of surface lands within 200 feet of the affected <br />land boundary were notified as evidenced by the signed return receipts from Certified <br />Mailings. Theses copies can be found in the enclosed Revised 112 Reclamation Permit <br />Application, Proof of Notifications Exhibit. <br />2. The maps have been signed by John Rozelle, Lafazge Land Manager, and Certified <br />Professional Geological Scientist. <br />3. Adjacent landowners identified as Cooley Gravel Co. and Western Paving Construction <br />Co. have been changed to Aggregate Industries, WCR and Lafazge West, Inc., <br />respectively. <br />4. Exhibit C has been revised to include the location of property owned by Gene Steve <br />Hardison (Parcel #131316000013). <br />5. Water lines located underneath Weid County Road 20'h have been added to Exhibit C, <br />documentation of notification to the owner, Left Hand Water District, can be found in <br />Proof of Notifications Exhibit. <br />6. The name and address of the owner of any road located within 200 feet of the affected <br />land has been added, as a structure owner, to Exhibit S.
