Long <br />Division Director <br />The Division has received your June 30, 2000 Regular 112 Application for the "Milliken Site" <br />submitted on behalf of Aggregate Industries, Inc. Listed below are adequacy review comments. <br />Since the recommended decision date for your application is scheduled for September 28, 2000, <br />the Division would appreciate a response from you on or before September 15, 2000. Please be <br />advised that the Division may recommend denial of your application on September 28, 2000 <br />unless the following adequacy review items are addressed to our satisfaction. <br />EXHIBIT C -PRE-MINING & MINING PLAN MAP <br />1. As required by Rule 6.4.3, please revise the Existing Conditions Map (1 of 3) to show the <br />owner's name of all irrigation ditches laterals and gas & oil structures contained on the area of <br />affected land and within two hundred (200) feet of the affected land. <br />EXHIBIT D -MINING PLAN ~ ?~ na ~~ ~,Laa~ <br />2. s required by Rule 6.4.4(d), please clazify the maximum disturbance to be expected during <br />f -th life of the operation.
