The enclosed <br />Sundry Notices are to document and inform the BLM that Natural Soda, Inc. has <br />completed the P/A activity. The EPA required the P/A work under the corrective <br />action section of the UIC Permit. Also enclosed are EPA Plugging Record forms, <br />P/A activity summary sheets, and well diagrams for each well. <br />-, <br />The goal of isolation of aquifers and cementing wells back to surface was <br />accomplished. Each well presented unique completion requirements and minor <br />variations to the proposed P/A plan were implemented. Per discussion with BLM <br />and EPA representatives, modifications to proposed plans were made. BLM and <br />EPA representative asked that approved variances be documented in this completion <br />notification. Specifically, the following actions were performed: <br />91-1 V-The first variance was setting the CIBP at 1860 FFS rather than 1920 FFS <br />due to obstructed well bore. Perforated at 1380 and later at 1250 FFS, pressured up <br />and verified competent cement above the A-Groove Aquifer. The second variance <br />was to obtain approval to cement well bore to surface and backside to surface. <br />93-3V-Obtained BLM and EPA approval to run CBL, and cement according to <br />results. NSI ran a CBL to determine top of cement. Based on the CBL, adequate <br />cement was found above and below the B-Groove. Perforations were made at 1250 <br />FFS with communication to surface. Cement was squeezed to surface, and cement <br />was run inside the casing to surface. Annulus was filled with cement from surface. <br />
