I have <br />listed the concerns below; please respond to each of these concerns <br />specifically. <br />Application Form <br />Item No. 4: Please submit address and phone number of general office; if it j <br />is the same as the local office please state so. <br />Item No. 5: Can Mr. Musick be contacted at the local office number? / <br />Item No. 13: The application fee is only $115.00. The Division will be <br />sending you a refund of $220.00. <br />Item No. 14: What is the county zoning? Is it agricultural? <br />Item No. 15: If it is zoned agricultural, will that be the final land use? <br />If a building is to be constructed for final reclamation or land use the <br />Colorado Mined Land Reclamation Board has required that a building permit be <br />provided as documentation. Your letter from La Plata County does not mention <br />approval for a building site. <br />Exhibit A <br />Please locate the property more specifically by quarter-quarter section / <br />location. If it can be broken down further, please do so. This same <br />description should be attached to your required warranties. <br />Exhibit B <br />Please be more specific in describing your operation. What type of equipment <br />will be used for hauling? Will there be crushing and screening or an asphalt <br />plant? If so, please locate these facilities on a map.
