(hr August 28, 1995, the Division directed the operator to repair all tailing
pond liner flaws and defects using an extrusion welding apparatus. The
1ivision further directed the operator to vacuum test all repairs, and to
provide the Division with a set of technical specifications detailing the
repair and vacuum testing procedures to be employed. In consultation with the
operator, a deadline of November 1, 1995 for completion of the repairs was
established. During a Discussion Item convened on November 15, 1995, the Board
«as notified that the repairs had not been completed. The Board then Ordered
the operator to complete the liner repairs by November 20, 1995. At the time
of the Division's December 11, 1995 site inspection, although the most critical
:Liner work had been completed, there were still liner flaws that required
patching. Days of violation at this time would be from November 1, 1995
through December 14, 1995, fora total of 43 days.
