Specialist: Allen Sorenson <br />6th PM, Existing Bond: $46,215.00 <br />t7ature and Duration of Violation: <br />(hr August 28, 1995, the Division directed the operator to repair all tailing <br />pond liner flaws and defects using an extrusion welding apparatus. The <br />1ivision further directed the operator to vacuum test all repairs, and to <br />provide the Division with a set of technical specifications detailing the <br />repair and vacuum testing procedures to be employed. In consultation with the <br />operator, a deadline of November 1, 1995 for completion of the repairs was <br />established. During a Discussion Item convened on November 15, 1995, the Board <br />«as notified that the repairs had not been completed. The Board then Ordered <br />the operator to complete the liner repairs by November 20, 1995. At the time <br />of the Division's December 11, 1995 site inspection, although the most critical <br />:Liner work had been completed, there were still liner flaws that required <br />patching.
