Sorenson <br />Reclamation Specialist <br />Division of Minerals and Geology VIA FAX AND U.S. MAIL <br />1313 Sherman Street, Room 215 832-8106 - 4 Pages <br />Denver, Colorado 80203 <br />Re: Mining Permit M-94-117 <br />Minerals Inspection Reports Dated 6/3/96 & 6/17/96 <br />Dear Allen: <br />Please find below a summary of corrective action taken as a <br />result of your inspection of the Gold Hill Mill on June 3 & 17, <br />1996. <br />June 3. 1996 Inspection Report <br />Item 1: A replacement pump was installed on the tailings pond <br />float on June 22, 1996. This pump replaced a pump <br />removed, without COM, Inc.'s permission, by the previous <br />operator of the mill. <br />While the pump was operational upon installation, the <br />spray lines on the tailings pond were plugged with debris <br />and had to be cleaned out. All are in operation at the <br />present time and will be operated a minimum of 40 hours <br />per week if sufficient water is in the pond to allow <br />operation. We have experienced ongoing difficulty with <br />the spray nozzles plugging but are taking corrective <br />action. <br />June 17. 1996 Inspection Report <br />item 1: See Item 1 above. <br />Item 2: The tear and other holes observed in the tailings pond <br />liner were repaired using approved methods by Colorado <br />Lining Company on or about July 1st. An inspection <br />report will be sent directly to DMG by Colorado Lining <br />Company. <br />item 3: Samples have been taken weekly since June 17, 1996 when <br />the spray irrigation system was in operation. Those <br />samples (two) are currently at the laboratory and results <br />will be forwarded to DMG upon receipt from the <br />laboratory. For most of this period the spray irrigation <br />
