Executive Summary <br /> 1. Introduction <br /> Ouray Silver Mines, Inc. (OSMI)is submitting this Amendment 2 application to the Division of <br /> Reclamation Mining and Safety (DRMS)to incorporate a proposed modification to OSMI's <br /> original Section 112(d) designated mining operation reclamation permit number M-2012-032. <br /> The following provides a summary of the proposed changes as well as an overview of permitting <br /> actions taken since 2012. <br /> The Revenue Mine is located in Ouray County approximately 6.9 miles southwest of the town of <br /> Ouray, Colorado. Silver, gold, and sulfide minerals are mined from vein deposits on patented <br /> mining claims owned by OSMI. Mining and milling are conducted entirely underground. Surface <br /> disturbance associated with the Revenue Mine operation includes surface facilities to support <br /> underground mining and milling activities, a five-stage passive mine water treatment system, and <br /> tailings storage facilities. The entirety of surface activities for the life of the mine will take place <br /> within the affected lands boundary near the Revenue Mine portal, all of which is located on <br /> patented claims within Ouray County, Colorado. The surrounding land is made up of either other <br /> patented mining claims owned by OMSI, a county right of way for County Road 26, and land <br /> controlled by the United States Forest Service (USFS). <br /> 2. Scope of Amendment 02 and Summary of Past Permit Changes <br /> Amendment 02 (AM-02) <br /> The scope of Amendment 02 is limited to expanding the affected lands boundary in Governor <br /> Basin to accommodate disturbance associated with construction of a refuge chamber and <br /> secondary escapeway. <br /> In addition to the proposed affected lands boundary modification in Governor Basin, the permit <br /> documents have been updated to incorporate changes made through various Technical Revisions <br /> (TRs)that have been submitted and approved since the permit was last amended in 2015. The <br /> following sections summarize the various TRs that have been submitted since the permit was <br /> amended in 2015. <br /> Amendment 01 (AM-01) <br /> In February 2015, an amendment was submitted to incorporate the Governor Basin mine area <br /> into the Revenue Mine permit. This area is located roughly 1.5 miles southwest of the Revenue <br /> portal area. Operations at Governor Basin was to consist entirely of excavating old ore piles and <br /> removing loose ore exposed on the surface. Additionally, the Revenue portal area permit <br /> boundary was expanded to allow more room for operational flexibility. <br /> Revenue Mine December 2021 1 <br />
