Portable sump pumps were deployed to <br /> pump slurry back into the circuit downstream of the failed equipment so it could be <br /> processed and discharged per the normal flow sheet. <br /> (b) any known or anticipated adverse impacts to human health, property or the <br /> environment; -There are no anticipated impacts to human health, property or the <br /> environment resultant of the mill event. <br /> (c) name(s), address(s), telephone and facsimile numbers of the Operator's contact <br /> person for additional information and follow-up by the Office; -Please contact Meg <br /> Burt, 100 N. 3`d Street, Victor Colorado, 80860, phone number 719-689-4055 <br /> (office), for any additional information on this subject. <br /> (d) monitoring and analyses that are necessary to evaluate the situation and corrective <br /> actions, copies of all pertinent data; and—The floatation dart valves causing the <br /> blockage and spill have been repaired and are functioning normally.
