Please be advised that if you are unable to satisfactorily <br />address any concerns identified in this review before the decision date, it will be your <br />responsibility to request an extension of the review period. If there are outstanding issues <br />that have not been adequately addressed prior to the end of the review period, and no <br />extension has been requested, the Division will deny this application. <br />1.6.2 General Applicant Procedures <br />1. As required by Rule 1.6.2 (1)(g), please submit proof of the public notice that was placed in a <br />newspaper of general circulation. <br />6.4.3 Exhibit C — Pre - Mining and Mining Plan Map(s) of Affected Lands <br />2. The map submitted for Exhibit C (General Layout with Existing Conditions) contains a note <br />(Note #4) which states that the reclaimed area will have a berm along the Big Sandy Creek. <br />This note contradicts the Reclamation Plan, which states that the berm will be removed <br />during reclamation. Please revise this map to reflect the Reclamation Plan. <br />3. The map submitted for Exhibit C (General Layout with Existing Conditions) specifies the <br />size of each phase. The three phases identified on the map cover a total of 127.3 acres. <br />However, the map and the application form state that the permit area is 108.04 acres in size. <br />Please clarify this discrepancy and revise the size of the phases so that they equal the permit <br />area. <br />6.4.6. Exhibit F — Reclamation Plan Map <br />4. The Reclamation Plan Maps (P1, P2, P3) identify three phases that cover a total of 127.3 <br />acres. However, the application form states that the permit area is 108.04 acres in size.
