PERMIT #: P- 2011 -014 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: August 15, 2013 <br />All eight (8) of the planned drill pads have been constructed. Henderson was not able to access the planned <br />location for drill pad #3. The pad was relocated along the road between drill pads #2 and #4. Henderson now <br />thinks either drill pad #6 or #7 will not be needed. The path of the access road between drill pads #7 and #8 <br />was altered after determining the original road was too steep. The additional road construction will be <br />reclaimed with the rest of the road reclamation in the future. <br />One hole was drilled and cased to a depth of 1120 feet on drill pad #8. The hole will be cored in 2014. The top <br />of the casing was observed during the inspection. The water required to core will be trucked to drill pad #5, <br />stored in tanks and pumped in a pipeline along the road to pad #8. Drill pads #6 and #7 were observed. No <br />drilling activity has occurred at the pads. One (1) hole was drilled and abandoned with bentonite grout and <br />cement on drill pad #5 in 2011. The abandonment report for the hole was included in the 2013 Annual <br />Report. No evidence of the hole was observed during the inspection. One (1) hole was drilled and abandoned <br />on drill pad #1. The pad has been partially reclaimed. The abandonment report for the hole was included in <br />the 2013 Annual Report. No evidence of the hole was observed during the inspection. Four (4) holes were <br />drilled on drill pad #2 in 2012. The abandonment reports for the holes were included in the 2013 Annual <br />Report. The pad was used for equipment storage during the inspection. One (1) hole was being cored during <br />the inspection on drill pad #3. The coring rig, support equipment and a lined sump were observed during the <br />inspection. One (1) hole was being drilled during the inspection on drill pad #4. The drill rig, support <br />equipment and unlined sump were observed during the inspection. The hole will be cored during 2014. <br />Photographs taken during the inspection are attached. <br />Henderson plans to submit a modification to the NOI during the winter of 2013. The modification is required <br />to extend the expected date of completion. The location of drill pad #2 and #3 will need to be clarified, drill <br />pad #6 or #7 may be removed from the NOI and an additional drill pad may be added near the Urad project. <br />As cited as a problem on page 1, the Division's files indicate Henderson did not submit an annual report and <br />fee for the NOI in 2012 for the 2011 -12 activity.
