PERMIT #: M -1977 -342 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: June 20, 2013 <br />southwest pit slopes have been regraded to approximately a 3H:1V slope. <br />Two areas of exposed groundwater were present in the pit, along the north end of the pit and along the <br />eastside of the pit in the northeast corner of the excavation. According to the letter dated September 30, <br />2011 from W. W. Wheeler and Associates, Inc., approximately 2.64 acres of groundwater were exposed prior <br />to December 31, 1980. As indicated in the letter dated November 8, 2011 from the Division of Water <br />Resources a well permit and augmentation plan are not required if there is to be no further use of the <br />groundwater and the pre -law amount of exposed groundwater is not exceeded. <br />NMGW -1: <br />Henderson notified the Division on June 4, 2013 of an NPL exceedance for pH at groundwater point of <br />compliance (POC) well MNGW -1 downgradient of the Henderson Mine. During the inspection, Henderson <br />confirmed the required second test result also exceeded the NPL. Henderson stated the required additional <br />data evaluation, data distribution and trend analysis will be performed and a report will be sent to the <br />Division. <br />Henderson expects to submit two technical revisions this year to revise the reclamation seed mixture and the <br />Tailings Delivery Line (TDL). The revised seed mixture will include more native species.
