PERMIT #: M -1977 -342 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: June 20, 2013 <br />OBSERVATIONS <br />The Henderson Mine was inspected by Peter Hays with the Division of Reclamation, Mining and Safety <br />(Division) as part of the Division's monitoring inspection program. Mr. Bryce Romig, Mr. Tim Haynes, Mr. <br />Miguel Hamarat and Ms. Amber Moran with Climax Molybdenum — Henderson Operations (Henderson) were <br />present during the inspection. <br />The inspection began with a meeting to discuss the permit status of the site. The following topics were <br />discussed: the recent process water release at the 3 -Dam, the fire water line spill, the tailings release from <br />Drop Towers 4 and 5, the tailings release from Drop Tower 7, the out of compliance pH level reading at <br />MNGW -1, the status of the revised bond estimate and structure of the financial warranty, the pump barge <br />relocation progress, the gravel pit status and the status of the active NOls associated with the site. <br />Financial Warranty: <br />Henderson is waiting for the Colorado Mined Land Reclamation Board to make a determination on the <br />financial warranty structure proposed for the Climax Mine. The Henderson site will request a similar financial <br />warranty if the Climax warranty structure is approved. The Board is expected to hear the item during the <br />August 2013 meeting. <br />Barge Pump Relocation: <br />The Barge Pump relocation was approved by the Division on April 8, 2013 as TR -19. Work on the relocation <br />was progressing. The barge inlet was constructed and road construction and pipeline fusing were occurring <br />during the inspection. <br />Drop Towers 4, 5 and 7: <br />The repairs to Drop Towers 4, 5, and 7 were observed by the Division. Henderson stated the Tailings Delivery <br />Line would be integrity tested and evaluated this year with possible replacement next year due to the recent <br />failures of the system. <br />3 -Dam process water release: <br />The 3 -Dam process water release area was observed by the Division. The area was dry, however evidence of <br />the release was observed from the lower pump vault and down the access road to the northeast. Henderson <br />stated a vacuum truck was used to clean up the pooled water on the road. Henderson stated the procedure <br />for the 3 -Dam seepwater gravity line pressure test would be changed to prevent a similar incident from <br />occurring in the future. <br />Fire Water Line release: <br />The Fire Water Line release area was observed by the Division. The broken pipeline was exposed during the <br />inspection. The pipe repair was delayed by the presence of asbestos insulation around the pipe. The repair <br />work was expected to be completed soon. Mr. Haynes stated pictures of the repair pipeline would be sent to <br />the Division. <br />Gravel Pit: <br />The gravel pit was inactive at the time of the inspection. 100,000 tons of material is planned to be removed <br />from the pit this year. Mining will continue to the north, approximately 600' of material remains at the north <br />end of the site. Topsoil and overburden stockpiles were observed south of the pit.
