Letter to Nick Michael 2 June 24, 2013 <br />AM -01 Application Adequacy Review File No. M- 1990 -057 <br />Designated Mining Operation (DMO) on November 23, 2011. Was the DOW aware, at the <br />time their comments were issued, that the facility would operate as a DMO? <br />4. Section 3.4 states the soil at the site is generally less than 6 inches thick. However, Section <br />3.7 states the soil is less than 5 feet thick. The Division assumes this was typographical error, <br />please clarify this discrepancy. <br />6.3.3 Exhibit C — Mining Plan <br />5. The Division is currently reviewing Technical Revision No. 4 (TR -04) which addresses <br />changes to the TSF liner and leak detection system. Please revise all applicable portions of <br />Exhibit C that change as the result of the approval of TR -04. <br />6. Please provide a flow diagram which illustrates the pathway through the various mill circuits. <br />In addition, specify the dimensions of all pipes and conveyances and specify the type of pipes <br />to be used, e.g. HDPE or double - lined. <br />7. The Applicant has stated ore will be placed into bunkers prior to feeding the crushing circuit. <br />Please provide the Division with a detail drawing and specification of concrete thickness and <br />reinforcing in order to evaluate the bunker both as an environmental protection facility and <br />for the cost of demolition. In addition, specify the total volume of ore the bunkers can store <br />and the location of the bunkers; the bunkers must be illustrated on Figure 6 -2. Please note <br />that the bunkers must be designed to not come into contact with stormwater during a 100 <br />year, 24 -hour rainfall event. <br />8. The Applicant has stated that thickened underflow from the tailings thickener will be sent to <br />the TSF in an accessible pipe in a concrete trough. Please illustrate the trough and pipe on <br />Figure 6 -2 and provide the Division with a detail drawing of the conveyance. <br />9. The Applicant has stated that containment loss of the stored chemicals or reagents will be <br />directed to the mill sump and then to the TSF. Please provide a description of fluid <br />management and describe how chemicals or reagents that are collected in the mill sump will <br />be conveyed to the TSF. <br />10. Please specify when construction of the new crushing building and attending facilities will <br />commence. <br />11. The Applicant plans on constructing a new crushing facility, a scale, a trailer foundation and <br />ore storage bunkers. Please specify the depth to which soil will be salvaged for use in the <br />reclamation process as Required by Rule 6.3.3(1)(b). In addition, describe how salvaged <br />topsoil will be stabilized and protected from erosion and depict the location of any new <br />topsoil stockpiles on the Figure 6 -2. <br />12. As required by Rule 6.3.3(2)(c), please describe the measures used to divert upland drainage <br />away from the site both during and after the operation. Also, include design details <br />
