Mr. Peter Hays, DRMS <br />June 18, 2013 <br />(d) No monitoring or analyses were performed for this situation since the water from 3 -Dam <br />was not released off -site and did not impact any natural water body. <br />(e) As of June 18, 2013 a root cause analysis was performed around the release. The <br />primary issue surrounding this release was administrative. There were two documents <br />associated with both systems that did not reference each other. Therefore, the work <br />instruction to complete the pressure test for the 3 -Dam Seepwater Gravity Line did not <br />reference the fact that two valves needed to be closed prior to backing water up into the <br />pipeline. A secondary finding was that check valves connecting the two systems did not <br />function as anticipated. <br />As action items from this event, Henderson will be addressing the administrative gaps <br />and will also develop a maintenance plan for the valves /check valves associated with this <br />system.
