Dear Mr. Hays:
Henderson Mill is providing this written follow -up notice to a phone call made to you on June 11, 2013 at
approximately 3:30pm pursuant to Rule 8.1 and 8.2 of the DRMS Rules and Regulations.
This notice is regarding a release of water originating from 3 -Dam process water which was inadvertently
backfed into the 3 -Dam Seepwater Collection System. The release incident was discovered on June 11,
2013 at approximately 12:30pm. While performing a required annual pressure test on the 3 -Dam
Seepwater Gravity Line, which delivers water from the toe of 3 -Dam to the toe of 1 -Dam, water backfed
into an intersecting system, the 3 -Dam Seepwater Collection System, which caused its lower pump vault
to fill with water and release approximately 500 -1000 gallons.
The following paragraphs correspond to the elements of the follow -up written notice set forth in Rule
8.2.3, paragraphs (a) through (e), of the DRMS Rules and Regulations:
(a) After discovering the issue, Henderson employees promptly terminated the pressure test,
turned off their water source and released the pressure from the valve at the lower end of
the pipeline. The release lasted approximately 10 minutes until noticed by the acting
operator.
(b) The water release did not adversely impact human health, property or the environment.
This is based on the known chemistry of the water and the location of the event. Water
pooled around the point of release (i.e., in the immediate area of the lower pump vault to
the 3 -Dam Seepwater Collection System) but did not flow from the property or impact
waters of the United States.
