Exhibit 13-27 of the permit contains certified as-built <br /> plans for each stock tank. The pond appears to be the proper configuration. All of the proper documentation has <br /> been submitted for this stock tank to be left as a permanent impoundment. <br /> Ponds 006, 009 and 015 were inspected. The configuration of these ponds was checked during the May 14, 2013 <br /> inspection. Please see that report for details. These ponds were holding water and discharging at the time of this <br /> inspection. The embankments were stable and vegetated and the primary and emergency spillways were clear and <br /> functional. <br /> ROADS —Rule 4.03; Construction 4.03.1(3)/4.03.2(3), Drainage 4.03.l(4)/4.03.2(4), Surfacing and <br /> Maintenance4.03.1(5) and(6)/4.03.2(5) and (6), Reclamation 4.03.1(7)/4.03.2(7): <br /> The following road areas within the site are under consideration for Phase I Bond Release; Mine Entrance <br /> Road/Routt County Road 53, Oil Well Access Road/Routt County Road 53,Haul Road A,Haul Road J and the <br /> only remaining portion of Haul Road K that has not been Phase I bond released. Along each road the Division <br /> took several measurements to verify the roads were constructed in accordance with the approved configuration in <br /> terms of width and grade. The width of each road was measured with a tape measure and the grade was checked <br /> with an inclinometer. Exhibit No. 20-3 in the permit depicts the approved post mine road profile and <br /> configurations for these roads. <br /> The Oil Well Access Road/Routt County Road 53 appears to have been recently surfaced with crushed rock. <br /> Exhibit No. 13-25.2 depicts the post mine plan and profile for this road. The Division measured with width and <br /> grade of this road at the OW-1 culvert. The road was found to be 25 feet in width and at about a 7%grade. The <br /> Division interpolated the grade where this culvert is located based on Exhibit No. 13-25.2 and found it to be 6.2%. <br /> The slight difference in grade is allowable. The surface of the road was recently graded and maintained. The road <br /> appears to be draining properly and large rills and gullies were not observed along this road. <br /> For Road A,the approved post mine width is 20+/-feet. The Division measured the width and the grade at four <br /> points along this road. One point is about 125 feet east southeast of grade stake 77+00 as depicted on the <br /> approved post mine topography map (Exhibit 20-2). The width of the road at this point was 21 feet and the grade <br /> was about 9%. The Division interpolated the approximate grade of this road based on the Exhibit No. 20-3 road <br /> profile and found it to be about 6.7%. This slight difference in grade is allowable. Another point was collected <br /> about 160 feet east of the 60+00 grade stake. The road was about 21 feet wide and at an 8%grade. The Division <br /> interpolated the grade at this point based on Exhibit No. 20-3 and found it to be about 6.7%. This slight difference <br /> in grade is allowable. Another point was collected where Culvert A-3 is located. The road was 20 feet wide at <br /> this point and at an 8%grade. Based on Exhibit No. 20 31,the grade at this point was 6.1%. This slight difference <br /> in grade is allowable. Another point was collected about 30 feet west of grade stake 15+00. The road was found <br /> to be 21 feet wide and at a 7%grade. The Division interpolated the grade at this point based on Exhibit No. 20 3 <br /> and found it to be about 5%. This slight difference in grade is allowable. The surface of the road was recently <br /> graded and maintained.
